New Patient
The day of your appointment please bring
• Government Issued ID or Photo ID
• Insurance Card
• List of any prescription or over-the-counter medications you are currently taking.

Financial Policy
All co-payments, deductibles and non-covered services are required to be paid at the time of service.
Prescription Refills
When requesting refills, please be prepared to provide us with:
• The name of the medication and dosage
•Pharmacy name
Please allow 24 to 48 hours for your prescription refill to be reviewed and phoned in. As a general rule, we do not process refill requests after hours or on the weekends.
Insurance
We accept most major health insurance plans. Please contact our office to confirm our acceptance of your plan.
